The DaVinci Center for Community Progress is a nonprofit organization serving the North End of Providence, Rhode Island. The center offers a variety of programs, including adult education, youth development, senior services, healthcare access, and community support, aiming to uplift residents through education, wellness, and community engagement.

Job Summary:

The Community Health Worker (CHW) is a trusted community member who provides senior citizens with culturally appropriate health education, outreach, and social support. The CHW will work closely with healthcare providers, social service agencies, and community organizations to improve the health outcomes of senior citizens by connecting them with necessary health services and resources.

Key Responsibilities 

Outreach and Education:

  • Conduct outreach activities to identify seniors in need of health services.

  • Educate seniors on health topics, including chronic disease prevention, mental
    health, nutrition, and wellness.

  • Facilitate health education workshops, support groups, and informational
    sessions.

Case Management:

  • Assist seniors in navigating the healthcare system, including making
    appointments, understanding medical instructions, and accessing services.

  • Conduct needs assessments to identify barriers to care and develop individualized
    care plans.

  • Provide follow-up support to ensure seniors are linked to appropriate services and
    that their health needs are being met.

Referral and Resource Coordination:

    • Develop relationships with local healthcare providers, social service agencies, and
      community organizations.

    • Refer clients to healthcare providers, social services, and community resources,
      such as food pantries, housing assistance, and transportation services.

    • Track and document referrals and outcomes using case management software.

Program Promotion and Community Engagement:

    • Promote health programs and services through community events, social media,
      and other outreach strategies.

    • Engage seniors in program activities and encourage participation in health
      initiatives.
    • Collect and report data on community needs, service utilization, and program
      outcomes.

Documentation and Reporting:

    • Maintain accurate records of client interactions, services provided, and outcomes
      achieved.

    • Prepare reports on outreach activities, program participation, and health outcomes
      for internal and external stakeholders.
    • Participate in program evaluation and quality improvement activities.

Qualifications:

    1. Education: A high school diploma or GED is required; an associate’s or bachelor’s
      degree in public health, social work, or a related field is preferred. Community Health
      Worker Certification in Rhode Island is required.

    2. Experience: Prior experience in community outreach, case management, or healthcare
      services is strongly preferred. Familiarity with the community being served is essential.
    3. Skills:
      * Strong communication and interpersonal skills, with the ability to build trust with
      clients and community members.
      * Knowledge of local health and social service resources.
      * Ability to work independently and as part of a team.
      * Organizational skills and attention to detail in documentation and reporting.
      * Basic computer skills, including familiarity with case management software or
      databases.

Other Requirements:

  • Language Skills: Bilingual English/Spanish required.
  • Transportation: A valid driver’s license and reliable transportation may be required for
    outreach activities.

Working Conditions:

  • This position may involve fieldwork, including home visits, community events, and
    travel to local healthcare facilities.
  • Occasional evening and weekend work may be required to attend community events or
    conduct outreach activities

Compensation: $20.00 per hour and health insurance
