General Job Description:

The Community Health Worker (CHW) provides navigational case management and community-based services to North End of Providence community members. The DaVinci Center for Community Progress seeks a compassionate and dedicated Community Health Worker (CHW) to support our mobile health clinic, which provides essential healthcare services to underserved residents of Providence’s North End of Providence. The CHW will be a critical link between community members and healthcare services, focusing on Alzheimer’s and dementia screenings, mental health support, and general health education. This role is crucial to improving community health outcomes by providing health resources, support, and navigation to individuals facing barriers to accessing traditional healthcare.

Major Duties and Responsibilities 

Under general supervision, the CHW is responsible for performing the following essential job functions: 

  • Recruit patients by conducting outreach activities in the community.

  • Builds trust with patients and community members to provide support, empowerment, education, and case management services.

  • Advises patients and others regarding health care and other community services available to them; assists patients in utilizing services; makes follow-up contacts when required.

  • Conducts periodic assessments of health behaviors such as patients’ physical activity, dietary habits, cigarette smoking habits, social factors such as housing and employment status, and social and economic resources.

  • Establish trusting relationships with patients, families, and community members to effectively connect them with health services. 

  • Conduct outreach activities to increase awareness and participation in mobile health clinic services, focusing on the senior population. 

  • Educates clients with chronic illness about evidence-based standards of care and self- management of their chronic disease. 
  • Documents work with patients through appropriate record keeping that follows the clinic’s policies and procedures; assists with gathering data relevant to program evaluation as appropriate. 

  • Participates in team meetings and assists in any activities related to the mobile clinic. 

  • Support the implementation of health education workshops, support groups, and wellness activities focused on mental health and chronic disease management. 

  • Assist in evaluating program effectiveness through patient surveys and feedback, using tools like the “My Senior Center” database for reporting and documentation. 

  • Serves as liaison between the professional staff and the community, developing relationships with various stakeholders in the community. 

  • Attends ongoing training for community health workers. 

  • Demonstrate privacy and confidentiality, meeting all the HIPPA regulations.
  • Works nights and weekends as required.

  • Other duties as assigned.

Qualifications For The Job:

Education: 

  • Community Health Worker Certification in Rhode Island 

Experience:

Two (2) years of verifiable experience providing information education intervention and referral services to culturally diverse populations are preferred. Recent experience (last five years) working as part of a multidisciplinary team in a healthcare setting is also preferred. 

Other:

  • Experience with motivational interviewing, advising/counseling clients, and participating in health promotion and health education activities. 

  • Excellent verbal and written communication skills. 

  • Excellent organization, thorough record-keeping follow-through, and ability to juggle multiple priorities in a fast-paced environment with numerous collaborators.

Physical Requirements:

  • Ability to work in a mobile clinic environment, including standing for extended periods, assisting with clinic setup/breakdown, and occasional lifting of supplies (up to 20 lbs).
